Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit Physical Therapy assessments, providing direct patient care and assessing medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation, experience with Hello Note is a plus.
• Hold a CPR Certification.
