Responsibilities

• Conduct home visit Physical Therapy assessments to evaluate patients' medical conditions and functional capabilities.
• Provide direct patient care to assess limitations and potential for rehabilitation.
• Utilize EMR documentation for accurate record-keeping.

Requirements

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Proficiency in EMR documentation.
• CPR Certification.
