Responsibilities

• Conduct home visit assessments to evaluate patients' medical conditions and functional capabilities.
• Provide direct patient care and develop rehabilitation plans.
• Proficiently document patient information in our EMR system.

Requirements

• Graduated from an accredited Physical Therapy program.
• Unencumbered Physical Therapy license for New York.
• CPR Certification.
