Position Overview

The Licensed Practical Nurse (LPN/LVN) will deliver high-quality patient care, coordinate care plans, and ensure patient needs are met in compliance with regulatory standards. The role requires clinical assessments, documentation, and adherence to care policies.

Responsibilities

• Coordinate and deliver skilled nursing care following established care plans.
• Monitor and report patient condition changes to supervisors and physicians.
• Document all patient activities, care interventions, and responses promptly.
• Educate patients and families on disease management and self-care techniques.

Requirements

• Graduate of an accredited school of nursing.
• Current state license as a Licensed Nurse and CPR certification.
• Proficiency in clinical assessments, documentation, and nursing care compliance.

Preferences

• Six months of recent clinical experience as a Licensed Nurse preferred.
• Home health experience is a plus.

Skills/Abilities

• Attention to detail and effective time management.
• Strong problem-solving and communication skills.
• Ability to handle physical requirements including lifting and prolonged standing.
