Responsibilities
- Deliver and coordinate quality patient care based on physician orders.
- Continuously assess patient condition and report changes to the supervisor.
- Document all activities, nursing actions, and care coordination accurately and timely.
- Participate in care plan development and achieve expected outcomes.
- Provide infection control and educate patients on self-care techniques.
Requirements
- Graduate of an accredited nursing school with current LPN license.
- CPR certification and proficiency in clinical assessments.
- Preferably six months of recent clinical experience and home health experience.
- Strong attention to detail, time management, and problem-solving skills.
Physical Requirements
- Must be able to lift 50 pounds, reposition patients, and move equipment.
- Ability to travel, climb stairs, walk, stand, and bend as needed.
- Visual, hearing, smell, and touch acuity required for patient care.
Environment
Must function in various environments, including exposure to allergens and infectious conditions. Possible exposure to blood and bodily fluids.
