Responsibilities
- Deliver and coordinate quality patient care based on care plans.
- Monitor patient condition, report changes, and take necessary actions.
- Document activities, assessments, and care provided in compliance with standards.
- Educate patients and families on healthcare needs and prevention strategies.
- Maintain infection control measures and adhere to regulations and standards.
Requirements
- Graduate of an accredited nursing school.
- Current, unrestricted LPN license in the state of practice.
- CPR certification.
- Proficiency in clinical assessments and documentation.
Preferences
- Six months of recent experience in a clinical care setting.
- Home health experience.
