Job Description
We are looking for a dedicated Registered Nurse (RN) Field Nurse specialized in Home Care to join our team. Duties and Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Develop patient-centered care plans consistent with available resources.
- Plan and execute patient discharge strategies.
Requirements
- BSN required; ADN candidates must complete BSN within three years.
- Current DE RN license or compact state license.
- Minimum of 2 years of recent Home Health RN experience.
- Proficiency in IV therapy or willingness to undergo training.

Work Schedule: Full time position with 20-40 hours per week, day shifts, occasional weekends, holidays, and on-call rotations.
