Registered Nurse (RN) - Home Health - $32-58 per hour

Job Description

We are looking for a dedicated Registered Nurse (RN) specializing in Home Health to join our team at Atrium Health in Charlotte, North Carolina. This position involves providing compassionate care to adult and geriatric patients in the comfort of their own homes, ensuring their well-being and quality of life.

Responsibilities

• Manage an assigned patient caseload, overseeing post-acute services in a home setting.
• Provide direct patient care, supervise LPNs and HHAs, and collaborate with a multidisciplinary team to meet patient needs.
• Ensure timely care, documentation, and coordination of services to promote optimal patient outcomes.

Requirements

• Graduate from an accredited School of Nursing, BSN preferred.
• Current RN license and BLS certification.
• Experience in home health care and case management is a plus.
• Strong critical thinking skills, ability to prioritize patient welfare, safety, and quality of care.
