Responsibilities

• Conduct home visit assessments and provide direct patient care to evaluate medical conditions and functional capabilities.
• Document patient information accurately in the EMR system.
• Work with compassion and dedication to facilitate patients' rehabilitation process.

Requirements

• Graduated from an accredited Physical Therapy program.
• Active Physical Therapy license in New York.
• Proficient in conducting physical therapy assessments and providing care.
• CPR Certification.
