Rehabilitation Therapist Responsibilities:

• Conduct home visits to assess patients and provide direct care.
• Evaluate medical conditions, functional capabilities, and rehabilitation potential.
• Document patient information accurately in our EMR system.
• Maintain CPR certification and adhere to professional standards.

Requirements:

• Graduate from an accredited Physical Therapy program.
• Hold an unencumbered Physical Therapy license in New York.
• Experience with EMR documentation preferred.
