• Conduct home visit assessments of referred patients to evaluate medical conditions and rehabilitation potential.

• Document patient progress accurately using EMR systems.

Requirements

• Graduation from an accredited Physical Therapy program.
• Valid Physical Therapy license in New York.
• Proficiency in EMR documentation is preferred.
• CPR Certification.
