Responsibilities
- Conduct home visit assessments to evaluate patients' medical conditions and potential for rehabilitation.
- Provide direct patient care to improve functional capabilities and quality of life.
- Document patient progress accurately using our EMR system.
Requirements
- Graduate from an accredited PT college or university.
- Hold an unencumbered PT license for New York.
- Proficiency in EMR documentation preferred.
- CPR Certification.
