Job Description
We are currently seeking a passionate Complex Care Social Worker - LMSW to join our dedicated team at Sinai Hospital, a part of LifeBridge Health in Baltimore, MD. As a full-time Complex Care Social Worker, you will play a crucial role in facilitating discharge planning for difficult-to-place, high-cost, and escalated patients. Your responsibilities will involve identifying and overcoming barriers to achieve effective discharges within specified goals while collaborating with the interdisciplinary team.
Responsibilities
- Utilize a multi-step escalation protocol to assess patients with discharge barriers.
- Collaborate with the interdisciplinary team to manage escalated patients and ensure compliance with discharge planning standards.
- Monitor difficult-to-place and high-cost patients for early intervention opportunities to facilitate timely and cost-effective discharges.
Requirements
- Master's degree or equivalent professional knowledge.
- Licensed Master Social Worker (LMSW) certification.
