Genesis HealthCare, a leading healthcare provider, is seeking a dedicated LPN MDS Coordinator to join our team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process, ensuring high-quality patient care and accurate reimbursement for services provided.

Position Highlights

• Direct the interdisciplinary Resident Assessment Instrument (RAI) process.
• Develop individualized resident Care Plans in compliance with Federal and State regulations.
• Train interdisciplinary team members on clinical content and MDS completion.

Requirements

• Current LPN license.
• Prior experience in healthcare settings.
• Knowledge of nursing practices, medical procedures, and long-term care regulations.
• Proficiency in computer applications, including AccuMed.
• Experience with MDS completion and Care Plans.
• Strong understanding of RAI/PPS processes.
• Current BLS/CPR certification.
