Job Description
We are currently looking for a dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing various challenges they may encounter. Your role will involve assisting with adjustment to institutional living, financial concerns, family relationships, medical understanding, discharge planning, and counseling related to death and dying.
Requirements
- Bachelor's degree in Social Work (BSW) required.
- Master's degree in Social Work preferred.
- LCSW-C preferred.
- LGSW certification required.
