Job Description
LifeBridge Health is seeking a compassionate Social Worker to join our team at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, families, and residents, addressing various issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life planning.
Responsibilities
- Conduct psychosocial assessments and provide counseling to address patients' needs.
- Collaborate with interdisciplinary teams to develop and implement care plans.
- Assist with discharge planning and community resource referrals.
- Offer support and guidance to individuals facing difficult life situations.
Requirements
- Bachelor's degree in Social Work (BSW) required; Master's degree preferred.
- LCSW-C preferred; LGSW certification.
- Experience in a hospital or healthcare setting is a plus.
- Strong communication and interpersonal skills.
