Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families. This role involves addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, discharge planning, and counseling related to death and dying.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Address common problems faced by patients and families.
- Collaborate with healthcare teams to support patients' well-being.
- Ensure timely and effective discharge planning.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- LCSW-C preferred.
- LGSW certification.
- Strong interpersonal and communication skills.
