Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker at Levindale Hospital, you will provide vital clinical social work services to patients and their families, addressing a range of psychosocial issues.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Address common patient concerns such as adjustment to medical procedures, financial worries, and end-of-life planning.
- Create discharge plans and offer support for patients facing challenging life transitions.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- Licensed Certified Social Worker (LCSW) designation preferred.
- Licensed Graduate Social Worker (LGSW) certification.
