Job Description
We are looking for a dedicated Social Worker to join LifeBridge Health in Baltimore, MD. In this full-time role, you will provide clinical social work services and psychosocial assessments to patients, addressing a range of common concerns related to their well-being and adjustment.
Responsibilities
- Conduct psychosocial assessments of patients and their families.
- Address issues such as adjustment to institutional living, family relationships, financial concerns, and more.
- Provide counseling and support for end-of-life care and transition planning.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- LCSW-C preferred, LGSW certification required.
- Prior experience in a healthcare setting is a plus.
