Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a full-time Social Worker, you will provide crucial clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, aging-related changes, and more.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Address common patient and family issues.
- Engage in discharge planning and provide counseling on end-of-life matters.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification required.
