Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health in Baltimore, MD. As part of our Allied Health division, you will provide clinical social work services and psychosocial assessments to patients and their families. Your role will involve addressing a variety of common issues such as institutional living adjustment, financial concerns, family relationships, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide clinical social work services.
- Assist patients with adjustment to medical diagnoses, treatments, and aging-related changes.
- Offer guidance on discharge planning and counseling for end-of-life issues.
- Collaborate with multidisciplinary teams to support patient well-being.
Requirements
- Bachelor's in Social Work (BSW); Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
