Job Description
LifeBridge Health is looking for a dedicated Social Worker to join our team at Levindale Hospital in Baltimore, MD. The successful candidate will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing a variety of common concerns such as adjustment to institutional living, financial issues, family dynamics, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Develop and implement discharge plans.
- Address issues related to aging, financial concerns, and medical treatments.
- Collaborate with healthcare teams to ensure comprehensive patient care.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification required.
- Previous experience in a healthcare setting is a plus.
