Job Description
We are looking for a dedicated Social Worker to join our compassionate team at LifeBridge Health. As a vital member of our Allied Health department, you will provide essential clinical social work services and psychosocial assessments to patients, residents, and their families. Your role will involve addressing a variety of common issues, including adjustment to institutional living, financial concerns, family relationships, medical procedures, aging-related changes, discharge planning, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments of patients and families.
- Provide counseling and support for emotional and practical needs.
- Collaborate with healthcare professionals to ensure comprehensive care.
- Assist in discharge planning and resource coordination.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification required.
