Job Description
LifeBridge Health is looking for a dedicated Social Worker to join our team at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients, addressing a range of common issues related to institutional living, family dynamics, medical procedures, aging, discharge planning, and end-of-life support.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Address financial concerns and assist with discharge planning.
- Support patients in coping with medical conditions and procedures.
- Collaborate with healthcare teams to enhance patient well-being.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C preferred; LGSW certification necessary.
- Experience in hospital or healthcare settings is a plus.
