Job Description
We are looking for a compassionate and dedicated Social Worker to join LifeBridge Health at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide clinical social work services, conduct psychosocial assessments, and offer support to patients, residents, and their families facing various challenges.
Responsibilities
- Provide counseling on adjustment to institutional living, financial concerns, family dynamics, and medical procedures.
- Assist patients with discharge planning and coping with aging and end-of-life issues.
- Collaborate with healthcare teams to ensure holistic care for individuals.
Requirements
- Bachelor's in Social Work (BSW); Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification required.
- Excellent communication and interpersonal skills.
