Job Description
LifeBridge Health is seeking a dedicated Social Worker to join our compassionate team at Levindale Hospital in Baltimore, MD. As a Social Worker, you will provide clinical social work services, conduct psychosocial assessments, and offer counseling to patients and their families. This role involves addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, aging-related changes, discharge planning, and end-of-life support.
Responsibilities
- Conduct psychosocial assessments and provide counseling services.
- Assist patients and families in dealing with medical diagnoses and procedures.
- Create discharge plans and offer support related to end-of-life issues.
- Collaborate with healthcare teams to ensure the well-being of patients.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- LCSW-C certification is preferred.
- LGSW certification.
